PAGE 41 Spotlight on “SeniorS” When returning home from the hospital or rehabilitation, there is often a period of adjustment. The news of going home may be viewed with mixed emotions of celebration and apprehension. A discharge home may prove to be challenging and over- whelming for the individual and their family members, especially if there are health issues involved. There are so many details to consider – new medications, physicians appoint- ments that need to be made, assistance with activities of day-to-day living such as show- ering, dressing or meal prepa- ration. All of these details can be very overwhelming for fam- ilies and left unaddressed can result in a poor transition to home for the patient. To help assist individuals to return to home safely and with a plan, Fellowship Helping Hands has recently established a program that addresses all of the necessary details that will help to ensure a seamless tran- sition back home. If home is living with a family member this will also help everyone adjust. Our transition team will devel- op a plan that includes an in-home assessment which includes a home safety check, review of medications, recom- mendations for home care, coordination of follow-up appointments with physicians and any additional assistance that may be needed.
