As the current Director of Nursing, it is my sole responsibility to ensure that our employees complete comprehen- sive assessments of our clients’ needs and limitations; and also develop a cus- tomized plan of care to ensure that the clients’ needs are met and the environ- ment is safe for our caregivers. To ensure this, our Registered Nurses complete these comprehensive assess- ments for each prospective client at the start of services and these initial assessments are used to determine an ongoing plan of care. The goals of the plan will be to assist in maintaining and/or restoring the client’s physical or cognitive abilities. These activities will allow the client to remain in the home situation and reduce the need for pre- mature institutional care. The plan of care is shared with the caregiver(s) assigned to the client as well as with the client’s family members. Further- more, our nurses conduct routine re- assessments of our clients and monitor all our caregivers during these re- assessments to ensure that the plan of care is adequately followed, the envi- ronment is safe, and provide addition- al instruction to our caregivers when needed. We also encourage our clients’ family members to participate in ensuring that the plan of care is fol- lowed and the environment is safe for all.
