Dr. Bhavti Soni is a pelvic health specialist Physical Therapist, practicing in New York City. She has been practicing for over 8 years and currently works with a leading team of pelvic pain surgeons, urologists, obstetricians and gynecologists. Her spe- cialties are endometriosis and autoim- mune conditions like Crohn’s & Colitis. As a founder and premier practitioner of Warren Physical Therapy, Warren, she believes in a multi-disciplinary approach for improved outcomes. Dr. Soni believes that pelvic health does not get the atten- tion it requires and a lot of patients she sees have been to multiple providers over many years. Many of her patients do not know what pelvic health/floor physical therapy is or they wonder about the reason why their MD has referred them to Warren. So let’s first talk about pelvic floor dys- function. Pelvic health dysfunction is an umbrella term and it comprises various conditions as bowel or urinary inconti- nence, pelvic pain, constipation, urinary or fecal urgency or frequency, bladder pain, tail bone pain, pelvic organ pro- lapse, pudental neuralgia and others. Pelvic health physical therapy is delivered by Physical therapists who are muscu- loskeletal experts and specialized in pelvic health dysfunction treatment. It includes a thorough evaluation including looking at the back, hip, posture and mechanics apart from the pelvic floor evaluation itself, which can be intravagi- nal or intrarectal. Treatment includes soft tissue manipulation which can be inter- nal or external, visceral manipulation, biofeedback, electrical stimulation, TENS, bladder/bowel retraining, or specialized exercise prescription. Many times there is a misconception that pelvic therapy is only for women, but it is for anyone who suffers from before mentioned symptoms including men and children. According to Dr. Soni, one of the major hurdles to get care for patients with abdomino-pelvic dysfunction is that the patients do not talk about their problems as they are embarrassed and the family and friends are not able to understand the symptoms. She finds a gap in the care process when it comes to pelvic health where the recovery is not followed through till the end. Just like we go to physical therapy after a rotator cuff or ACL reconstruction surgery we should get PT after hysterectomy or prolapse repair or vestibulectomy, prostatectomy or other abdmino pelvic laparoscopic sur- geries for post-surgical rehab. Postpartum care is another widely ignored area. The American College of Obstetricians and Gynecologists recently updated their guidelines giving tremen- dous boost to postpartum physicalthera- py and raising the bar for post-partum wellness. It states that post-partum women should contact their obstetric care providers within the first 3 weeks, recognizing that new mothers need ongoing care during the fourth trimester instead of a one-off checkup at 6 weeks.
