LIPS MADE EASY By: Andrea Strauss, M.D. F.A.C.S. Aesthetic medicine has its own unique vernacular. That extra skin under the chin is a turkey gobbler deformity. Skin hanging down from the upper arms is a batwing deformity. Some- times we are identifying the condi- tion and sometimes we are describ- ing the results of treatment. Perio- ral rhytids (vertical lines around the mouth) are called “smokers lines” or “barcode lines.” They are easy to name but hard to treat. Laser resurfacing can improve the lines but is painful, has a significant downtime and can lighten the skin. Botox can soften the lines a little and must be repeated every 2-3 months. Adding too much stan- dard filler to the lip vermilion (pink part) can result in “duck lips.” Adding standard filler just to the vertical lines can cause “chim- panzee lip.” (Again that fixation with animal themes). Galderma, an international phar- maceutical company concentrating in dermatology, has finally found the right treatment for those pesky lines and lipstick bleed, creating a natural look without excessive vol- ume. Restylane ® Silk is a thinner version of standard fillers (Resty- lane ® and Juvederm ® ). These fillers are made of hyaluronic acid, a sub- stance found naturally in the body. Since it is non-animal based, pre- testing is not necessary. Restylane ® Silk injected along the vermilion border (the white line between pink lip and skin) and into the vertical lines helps smooth the wrinkles while softly restoring lip shape and definition without over-plumping. Restylane ® Silk is injected in the office. The product is mixed with lidocaine (local anesthetic) so dis- comfort is minimal. Possi- ble side effects include mild swelling, tenderness, bruising and/or redness which can last 2-7 days. A cold compress is applied after injection to minimize swelling and bruising. Intermittent applica- tion of the cold compress during the day of injection will help limit the swelling. Bruising can be further minimized by avoid- ing NSAIDs (aspirin, ibuprofen, Naprosyn etc.) for several days prior to the injections and taking Arnica pills for 3 days before and 3 days after treatment. Restylane ® Silk should not be used in anyone allergic to lidocaine or with open wounds around the mouth. Those with a history of Her- pes Simplex (cold sores) should be pretreated with Valtrex or a similar medication prior to treatment. Due to the increased motion of the lips with talking and eating, Restylane ® Silk does not last as long as thicker hyaluronic acid fillers injected into the face, requiring additional prod- uct every 6-8 months.
